For IBD, diagnosis requires a combination of clinical findings, inflammatory laboratory markers, imaging findings and endoscopic biopsies. Your doctor is looking for visible evidence of inflammation or damage in the intestinal lining.
For IBS, doctors use the Rome IV criteria, a set of clinical guidelines. The Rome IV diagnostic criteria define IBS as recurrent abdominal pain occurring at least one day per week in the last three months, involving changes in defecation, stool frequency or stool form. No blood test confirms IBS and no scan that reveals it, making it a clinical diagnosis, which is exactly why ruling out IBD first is such an important step.
Treatment
This is where it truly matters that you know which condition you're dealing with, because the approaches are completely different.
IBD requires long-term medical management. Treatment typically involves anti-inflammatory medications, immunosuppressants and biologic therapies and some patients eventually need surgery. Because IBD involves active inflammation causing real structural damage, it cannot be managed with diet and lifestyle changes alone.
IBS, on the other hand, responds well to lifestyle and dietary interventions. The low-FODMAP diet has shown strong evidence for IBS symptom relief, with studies showing symptom reduction ranging from 55% to 89% following FODMAP reintroduction and personalization.
Stress management is also a major piece of the puzzle, since the gut-brain connection means anxiety and stress can directly worsen IBS symptoms. Regular physical activity can help manage IBS symptoms by keeping the digestive system on track. Probiotics, peppermint oil and cognitive behavioral therapy have also shown promise. Can You Have Both?
IBS cannot turn into IBD. They are entirely separate conditions with different causes and mechanisms. That said, it is possible to have both simultaneously and some people with IBD experience IBS-like symptoms even when their IBD is in remission, which can make things more complicated to manage.
On a scale of one to 10, IBS is far more common. IBS affects roughly 10 to 15 percent of adults, while IBD prevalence in the U.S. is estimated between 2.4 and 2.8 million people. IBS is not a lesser or imaginary version of IBD. It is a real condition with a real impact on daily life, even without structural damage.
When to See a Doctor
If you're dealing with ongoing digestive symptoms, getting checked out is always worth it rather than guessing. But certain red flags should prompt you to seek medical attention sooner rather than later: blood in your stool, unexplained weight loss, persistent fever or symptoms that wake you up at night. These are not typical IBS symptoms and they could signal IBD or another condition that warrants prompt evaluation.
Even if your symptoms feel more in line with IBS, a proper diagnosis from a gastroenterologist gives you a clear starting point and a path forward. Self-diagnosing based on internet searches is how people end up managing the wrong condition for years. Your gut deserves better than that.
